Pre-Operative Forms & Preparation
To ensure a smooth and seamless experience, we kindly request that all required forms and documentation be completed and submitted to:
We encourage clients to submit all required forms at least two weeks prior to their scheduled surgery date. This allows our team sufficient time to review your information, coordinate your care, and prepare for your individual recovery needs.
If you have any questions or require assistance with the paperwork or preparation process, our team is available to provide guidance and support.
To secure your reservation, a 20% non-refundable deposit is required at the time of booking, subject to the terms of your service agreement.
The remaining balance must be paid at least two weeks prior to your scheduled surgery date, unless otherwise agreed to in writing.
Please be advised that reservations made within the same week as the requested service date, as well as bookings during holidays, may be subject to additional fees based on availability, staffing, and service requirements. We encourage you to plan and reserve your care in advance to support the best possible coordination of services.
